The EAC program is designed to provide more intensive therapy for those who need it, in order that they may eventually return home and live comfortably without the constant supervision. Instead of just providing emergency room-level care with little commitment from clients or their families (which can often lead them back), this therapeutic environment encourages stability through long term rehabilitation efforts focused on community integration too--so even if youre not feeling better right now therell still be help available when your head hits pillow at night!

  • Application Process:1. A team meeting is held with the SW, Physician, County of originand the appropriate MCO to determine appropriateness for this levelof care. 2. EAC packet must be completed with all clinical informationcompleted
  • Eligibility Requirements:1. Must be a resident of Bucks, Montgomery, Chester or DelawareCo.2. Must be currently in an acute care setting.3. Must have a Serious Mental Illness (SMI)4. Must have been in an inpatient acute care program for at least45 days in a 12 month period.5. Must have Managed Medicaid for one of the 4 counties mentionedabove
